A procedure to correct the gum shape and contour thus improve the smile line. Gingivoplasty surgery is usually done on the upper front teeth (can be up to the premolar region) where the gums here can be seen when smiling.
Most of these gum defects are usually caused by gum diseases (eg. periodontitis). However, in certain cases, the gum recedes due to over brushing or probably just because the gum itself is thin (thin biotype).
In gingivoplasty, a gum graft can be done where the tissue is taken from the roof of the mouth (this is called a graft) and then stitched into place on either side of the tooth that is recessed.
Here, is a case of a young male presented with receded gum due to over brushing…
He didn’t like his teeth and gum for a few aesthetic
reasons:
The teeth are uneven
The gums on his left are simply too high!!
We found out that he was a right-handed person, using his toothbrush to brush his left teeth too hard!! That causing the gum to shrink upwards.
Secondly, his gums were thin biotype and the previous dentist had over-filled his abrasion cavity with composite preventing the gum from ‘coming down’
Objectives of treatment:
Correct his over-brushing technique
Modified the over-filled composite restoration
Correct the Shape of his teeth

Dr Joann Yong received her dental degree in 2011 and obtained her Membership of Faculty of Dental Surgery from the Royal College of Surgeons of Edinburgh, UK in 2014. She then undertook specialty training in orthodontics at the National University of Malaysia and was awarded Doctor of Clinical Dentistry with Distinction in 2020. Dr. Tham Ru Yueh received her Bachelor of Dental Surgery(BDS) from International Medical University(IMU) in 2015. She then completed her compulsory government service in Ipoh, Perak.
When she started working in private dental clinics in 2018, she realised that she loved treating children as it makes her feel very fulfilled. She saw that there was so much more that she can do for children other than preventive (polishing, fluoride varnish application, dental education) and restorative work (filling, stainless steel crowns) because more and more children are starting to have crooked teeth at a very young age. This led to her quest in seeking for answers and the beginning of her journey in interceptive orthodontics, where she now provides treatment for children in the mixed dentition stage (children at the age with permanent and baby teeth) to help expand narrow jaws, grow beautiful faces with natural smiles and achieve better airway health.
Apart from her interest in seeing children, Dr Tham is also excellent with orthodontics and esthetic restorations (fillings and veneers). She continues to attend many Continuing Dental Education courses and workshops in order to keep her knowledge up to date because she believes that her patients deserve the best care.
Dr. Chong is a general dental practitioner who received her Bachelor of Dental Surgery (BDS) degree from University of Malaya in 2008. She regularly attends braces, implant and aesthetic dental educational courses in Malaysia to keep improving herself. With years of extensive experience and skills, Dr. Chong enjoys bringing compassionate and comprehensive dental care to her patients.
With an inherent passion to help people, she is keen in educating patients to attain good oral health and to promote self care approach in daily life to achieve optimal oral hygiene. Dr. S. N. Ng is a general dental practitioner. She graduated from University of Malaya in 2003.
She is the founder and director of Prestige Dental Care Sdn. Bhd. She used to work in Prestige Dental Care, Bandar Puteri Branch before moving to Prestige Smile Bright, Bandar Puteri. Now she is in-charge and leading doctor in Prestige Smile Bright.
She mainly interest is braces and aesthetic dentistry which include teeth whitening, crowns and bridges. She attended many courses to improve her skills in denstistry field; and now she is one of the Invisalign Provider which provide clear aligner to align teeth.

Dr. H. W. Ng is an oral & maxillofacial surgeon. He obtained his basic degree as a dental surgeon from University of Malaya in 2001 and become the member of the Faculty of Dental Surgery, The Royal College of Surgeons of England in 2006. He passed his master degree, MClinDent (Oral & Maxillofacial Surgery) from University of Malaya in 2009.
Dr. H. W. Ng has more than 15 years of working experience as a general dental practitioner and 10years as an Oral & Maxillofacial Consultant. He worked as a oral & maxillofacial surgeon/trainee in a few centres in Klang valley, namely University Hospital, Hospital Tunku Ampuan Rahimah in Klang and Hospital Kuala Lumpur. Currently, he is the visiting Oral & Maxillofacial Surgeon in Assunta Hospital, visiting oral surgeon in a few dental clinics in KL and a part-time lecturer in the Oral & Maxillofacial Surgery Department in Dental Faculty, International Medical University (IMU) in Bukit Jalil.
He is the founder and director for Prestige Dental Care Sdn. Bhd. and currently, he works in Prestige Dental & OralFacial Surgery in Setiawalk. What is Digital Smile Design
DSD is a dental treatment planning tool that helps the dental providers to have improved diagnostic vision, better outcome predictability and improved communication between the providers and patients. With DSD, the dental providers would be able to design a better and beautiful smile customized to every patient and be able to deliver the treatment planning confidently to the patients. With the patients’ complete understanding, they could perform the dental treatments efficiently.
How does DSD work?
When the patients have decided to make their smile look better, the dental providers will communicate with them to understand their expectations and needs from the treatment. Then, the providers will take photos of them and their teeth. Later, all the required photos will be imported into the DSD software.
Then, the software will help the dental provider to analyze the patients’ dental and facial proprotions thoroughly. There are various forms and shapes that can be chosen from the digital library which suits best for the patient. This gives the patients a quick glimpse into what they can expect at the end of the treatment.
Next, the providers will explain to the patients the problem lists. A thorough treatment plan is created based on the problem lists and what dental treatments are needed to achieve the final outcome. The treatment can range from simple restorative procedures, orthodontic treatment to orthognathic surgery.
Case Sample
First, patient is required to take photos of their smile.
Next, photos are imported into the DSD software. The providers identify the patient’s smile issues. Then, the software is used to analyze their dental and facial proportion, thus designing a better smile design customized to every patient.
Lastly, the final outcome of the smile design is then displayed on the software. Treatment plan based on the design is then discussed with the patient.
What makes DSD unique?
Although digital imaging and diagnostic mock-up can show to the patients how will they look like at the end of the treatment, they have their limitations in terms predictability. With DSD, the providers are able to tell the patients exactly what their smile will look like because final outcome of the patient’s smile will be displayed in the software and the accuracy of the the final outcome is high. With these, miscommunication between the patients and providers can be avoided as they will be on the same page.
